Her area of professional specialization is in forming collaborations with leaders of hospitals, nursing homes, home care agencies, physician practices, and other health care and community providers to improve communications and processes for patient transitions between health care settings. Ms. Elwell was instrumental in starting the CT Partners for Health, a group of more than 40 stakeholders in the healthcare industry, and serves as the President of its Board of Directors.
